If you would like to register a participant into the program, please download, fill-out and submit the following forms which are all required for acceptance: RMYF Registration Form, Emergency Medical Consent Form, Image Release Form, Medical Clearance Form, Waiver and Release of Liability Form, No Tolerance Form.
